Enclosed is a proposed agenda and schedule for the 2005/2006 21st Century Community Learning Centers monitoring visit and the Monitoring and Self-Assessment Tool. The monitoring visit will include a review of your 21st CCLC records and the student files referenced in the Monitoring and Self-Assessment Tool, interviews with site directors, community partners and students, a tour of the facility and as well as discussion of any technical assistance issues or concerns. It is important that your program supervisor be available for at least part of this visit.
In preparation for the monitoring visit, please review the Monitoring and Self-Assessment Tool that is attached to this email as it has recently been revised. It will be used to guide discussion at the monitoring.
If you have any questions, please contact the monitor who is conducting your review. Thank you for your prompt attention to this matter. We look forward to seeing you.

21 Century Community Learning Center Monitoring Visit
The schedule below provides a format to conduct the 21 CCLC Monitoring visit. This format will assist the monitors in completing the monitoring form and will provide an opportunity to speak with individuals who can provide relevant information about the Center and its daily operation. Please schedule a private room for the monitoring discussions. Staff who are required to attend are identified with an *(asterisk). Site Coordinators are scheduled over lunch so they can be available to return to their site prior to the start of the after-school program. In 21 CCLC programs with multiple sites, monitors may wish to visit more than one site. The attached monitoring tool identifies the required materials under the documentation column.
